PEID Positioning and Fluoroscopic Localization: Building a Safe Puncture Path

view count:

WEB SUMMARY  Correct prone positioning and fluoroscopic localization determine the puncture direction, neural safety, and decompression reach in PEID. This article explains positioning, level confirmation, and puncture-point planning.

Positioning must serve both anesthesia and surgery

The patient is usually placed prone with the lower limbs lowered and the hips and knees flexed. This position can open the interlaminar window, reduce lumbar lordosis, and help the surgeon establish a working trajectory at the target level. Eye, neurologic, pressure-point, and perineal protection should be managed by the anesthesia and nursing teams.

Level localization cannot rely on surface landmarks alone

Fluoroscopy confirms the target interlaminar window and segment, using anteroposterior, lateral, or oblique views as needed. The puncture point is typically about one centimeter lateral to the posterior midline on the affected side, adjusted to the interlaminar space and herniation location rather than applied as a fixed measurement.

Puncture direction defines the decompression route

The needle is generally directed toward the interlaminar window medial to the affected facet joint, with fine adjustment according to the position of the herniated disc. The goal is to point the working channel at the compressive source while protecting neural structures.

Imaging and tactile feedback should agree

Fluoroscopy confirms bony position, but it cannot replace judgment about tissue layers and neural anatomy. If resistance, direction, or needle position differs from the plan, advancement should stop and the route should be reassessed. A clear understanding of the ligamentum flavum, lamina, and facet is the basis for safe puncture.

Reserve space for the dilation steps that follow

An ideal puncture not only reaches the target but also provides a stable axis for sequential dilation and working-cannula placement. Repeated adjustments increase soft-tissue trauma and radiation exposure, so preoperative imaging measurement and standardized marking reduce intraoperative re-puncture.
